Hemma  ·  At Home

Hemma is the Swedish word for "at home" — and that's exactly where we work. Alongside families, in the places and moments that matter most. We believe aging well isn't about where you live. It's about staying connected to the life you've already built, wherever that life takes you.

Two Ways Hemma Helps

After Hospital or Rehab

Post-Discharge
Stabilization

The 30 days after leaving a hospital, skilled nursing facility, or rehab are the most vulnerable. Hemma coordinates the transition home — medications, follow-up appointments, transportation, equipment, family communication, and provider briefings — so recovery has structure.

What we coordinate:

  • Medication confirmation & delivery
  • Follow-up appointment scheduling
  • Transportation coordination
  • Home equipment & safety setup
  • Family communication & updates
  • Provider briefings & closed-loop reporting

Who We Are

A Hemma coordinator walking alongside a client near his Colorado home.

We know the medical system from the inside — so your family never has to figure it out alone.

We also know what it feels like to be the family member in the waiting room, hoping someone qualified is paying attention. That's why we built this.

Our team spent careers inside the clinical, research, and healthcare infrastructure that families are expected to navigate on their own. We understand how hospitals make decisions, how insurance systems work, how moves between hospitals, rehab, and home go wrong — and most importantly, who in Colorado to call when your family needs the right person in the right moment.

Certified Senior Advisor (CSA)

The CSA credential is one of the most rigorous designations in aging services — requiring comprehensive education across health, financial, and social issues affecting older adults, and ongoing professional accountability. It means your family is working with someone formally trained in the full complexity of aging, held to a professional standard, and committed to continuing education in elder care. It's not a title we use lightly.

Clinical Care & Wound Care

Hands-on clinical experience in direct patient care gives our team a grounded, practical understanding of what happens inside the healthcare system — not just around it.

Clinical Research & Global Health

Graduate-level training in clinical research and global health policy sharpens how we evaluate medical options, interpret evidence, and advocate for treatments that are right — not just convenient.

Psychology & Behavioral Science

Understanding how people process fear, grief, and difficult decisions is central to everything we do. We're trained to hold hard conversations with both honesty and compassion.

Healthcare Innovation & Personal Caregiving

Experience as medtech investors, startup builders, and — most personally — as family members who have cared for aging loved ones ourselves. We understand this from every angle.

For Referring Partners

Discharge planners,
SNFs, physicians,
and fiduciaries.

Hemma gives your patients and their families a non-clinical layer of coordinated support after discharge — and a trusted ongoing resource for the families you work with long-term.

We don't overlap with your role. We close the coordination gap that exists between discharge and recovery — and we close the loop with you when it's done.

Make a Referral

Hospitals & SNFs

Hemma's post-discharge coordination helps reduce the common gaps that lead to avoidable readmissions. At the end of every 30-day engagement, referring partners receive a structured outcome report documenting what was completed and what was flagged.

Geriatricians & PCPs

Pre-appointment briefings mean your patients arrive prepared. Post-visit follow-through means the tasks you assign actually get done. Hemma is the coordination layer your practice doesn't have time to provide.

Elder Law Attorneys & Fiduciaries

We handle care coordination. You handle legal and financial matters. No overlap, no conflict — just a clean, trusted referral relationship that serves your clients well.

Frequently Asked Questions

What is geriatric care management?

Geriatric care management is professional coordination and oversight of care for aging adults and their families. A care manager helps navigate the medical system, manage logistics, communicate with providers, and plan ahead — without providing clinical or nursing services. Think of it as having a highly informed, professionally trained advocate who knows exactly how the healthcare system works and who to call in Colorado when something needs attention.

What is the difference between a geriatric care manager and a home health aide?

A home health aide provides hands-on physical care — bathing, dressing, and daily living support. A geriatric care manager provides coordination, oversight, advocacy, and planning. We don't provide personal physical care. We manage the full picture: the calendar, the providers, the insurance, the transitions, and the conversations that need to happen before a crisis forces them.

Who typically reaches out to Hemma Care Management?

Families reach out in a few different ways. Often it's an adult child — a busy professional, sometimes living out of state — who has realized that staying on top of a family member's care has become a part-time job. Sometimes it's a spouse managing the added complexity of a partner's health needs alongside their own. And sometimes it's the older adult themselves, who wants professional support in place to stay organized, informed, and independent. What most have in common: the system has started to feel unmanageable, and they know they need someone who understands it.

What does non-clinical care coordination mean?

It means we coordinate, communicate, research, and advocate — but we do not diagnose, treat, prescribe, or administer medications. We work alongside your family member's medical team, not in place of them. Our role is to make sure nothing falls through the cracks between appointments, providers, and family members — and that someone qualified is paying attention at every step.

What areas of Colorado do you serve?

We serve families throughout Colorado, with a primary focus on the Denver metro area and Front Range communities. If you're unsure whether we cover your area, reach out — we're happy to talk through your situation first.

How do I know if Hemma is the right fit?

A good fit usually looks like this: an aging family member managing at least one chronic condition, a family that is involved but stretched thin, and a recognition that professional coordination would meaningfully reduce stress and risk. If someone in your family is spending hours on hold with insurance, scrambling to understand what was said at an appointment, or lying awake worried about what might be getting missed — that's exactly the gap we fill. We offer a no-obligation conversation to help you decide.

What happens when we first reach out?

We start with a conversation — no forms, no pressure. We'll ask about your family member's current situation, what's working, what isn't, and what your family most needs right now. If it feels like a good fit, we schedule an in-home assessment to build a complete picture before coordination begins.

What types of support does Hemma offer?

Our services range from comprehensive ongoing coordination to more targeted support depending on what your family needs. Some families need full logistics management — calendars, providers, insurance, planning. Others need a physically present advocate in the room at appointments and during hospital discharges. We'll help you figure out what makes sense for your situation.

Do you work with couples?

Yes. When two people are navigating aging together, the coordination becomes significantly more complex — overlapping health needs, caregiving dynamics between partners, and shared decisions that affect both lives. We offer services tailored specifically for couples at both service tiers.

How does Hemma work with existing doctors and medical teams?

We communicate directly with your family member's medical team — primary care physicians, specialists, and care facilities — on your behalf. Before appointments, we brief providers on recent changes in condition, medications, and relevant history. After appointments, we make sure the family understands what was discussed and what follow-up is needed. We don't replace the medical team. We make sure your family gets the most out of every interaction with them.

Is Hemma covered by Medicare or insurance?

Geriatric care management services are generally not covered by Medicare or most private insurance plans. Our services are paid privately. That said, one of the things we do is help families maximize every benefit they're already entitled to — including Medicare Advantage dental, vision, hearing, transportation, and over-the-counter allowances that most families leave unused.

What credentials does your team hold?

Our team holds the Certified Senior Advisor (CSA) designation — one of the most comprehensive credentials in aging services — alongside backgrounds in clinical care, clinical research, global health, psychology, and healthcare innovation. We've spent careers inside the healthcare system, not just around it. That means we understand how hospitals make decisions, how insurance systems work, and who in Colorado to call when your family needs the right person in the right moment.
